Job Summary

A company is looking for an RN Care Manager to provide intensive nursing care management and collaborative consultation for high-risk members.

Key Responsibilities
  • Review and update member information to meet NCQA requirements
  • Facilitate comprehensive assessments and evaluate care effectiveness
  • Develop integrated care plans and engage members in their care
Required Qualifications
  • Minimum of two years of experience with the MH/SU/IDD population
  • Minimum of three years of experience as a Registered Nurse
  • Knowledge of NCQA preferred
  • Understanding of NCQA components and NC/Federal Regulations
  • Proficiency in Microsoft Office products
