Bilingual Care Coach (RN, LPN, LVN)

Job Summary

A company is looking for a Care Coach - Spanish Bilingual (RN, LPN, LVN).

Key Responsibilities

• Engage and maintain members in active participation in the health and wellness program
• Utilize motivational interviewing and skill-building techniques in communication with members
• Collaborate with care teams and provide feedback to enhance program quality

Required Qualifications

• Associate's or Bachelor's degree in Nursing preferred; equivalent education or two years of experience may be considered
• Extensive experience in a health care consumer-facing environment
• Experience performing assessments and implementing care plans
• Extensive experience in a clinical setting with Serious Mental Illness population
• Bilingual in Spanish with a requirement to pass a verbal language proficiency test
