Develops and monitors Tailored Care Management care plans and Individual Support Plans (ISPs) for assigned caseloads
Coordinates care team meetings and engages with members and families to ensure service provision and follow-through
Promotes comprehensive care by connecting members with various support services and addressing unmet health-related needs
Required Qualifications, Training, and Education
Two years of experience as a Care Manager, Case Manager, or Care Coordinator preferred
Must meet all agency requirements for pre-employment and those required by the state of NC
A license or certification in a relevant human service profession, or a degree in a human service field with specified experience
Experience with the IDD population is required, with specific conditions for LTSS needs
Ability to perform work with a high degree of quality and autonomy
