Job Summary

A company is looking for a Remote LPN Intake Coordinator with Michigan & Compact Licensure required.

Key Responsibilities

• Conduct preliminary health assessments for newly enrolled patients
• Resolve patient inquiries and coordinate community resources for care
• Provide education on chronic conditions and assist with medication management

Required Qualifications, Training, and Education

• Active Michigan & Compact/Multi-State LPN (LVN) license
• At least 3 years of direct patient-facing work experience
• Technical aptitude with Microsoft Office Suite
• Ability to work in a fast-paced environment
• Must have a home office or HIPAA-compliant workspace
