NJ Support Coordinator

Position Title: Support Coordinator
Location: Wrightstown, NJ

We are looking for a motivated, organized, energetic, technologically proficient individual that is not only good at writing and paperwork but also great with individuals with disabilities. On top of this, we are looking for someone who shares our passion for working with adults with disabilities specifically.
Support Coordinator Responsibilities:
  • Manage services for individuals with disabilities.
  • Conduct intake meetings.
  • Develop a plan for services based on the needs of the individual.
  • Facilitate the start of services for the individual with state vendors.
  • Monitor the individual and the plan on a monthly basis
  • On call for emergencies

Required Experience:
  • Bachelor’s degree in the field of Social Work, Psychology, Sociology, Counseling, Gerontology, Special Education or Rehabilitation.
  • Minimum of one year experience in the field of intellectual/developmental disabilities required
  • BOGGS Support Coordination Orientation Certificate preferred but not required
  • Basic computer skills with proficiency in Microsoft word to include entering, updating, and maintaining Consumer Information System data.
  • Must possess valid driver’s license
  • Must be able to provide proof of auto insurance.
  • Per Executive Order No.283 COVID-19 Vaccination's are required.
