What You’ll Do 

The Program Manager will oversee programmatic processes, requirements, and governance of Strive’s Comprehensive Kidney Care Contracting (CKCC) program, a CMS government model. This individual will drive the execution of key initiatives to support markets’ success in the program and ensure key CMS deadlines are met. Requires close collaboration and strong connections across various Strive Health teams and executives, as well as direct coordination with CMS. This leader will demonstrate strong program management, data analysis, project design, effective communication and subject matter expert skills. In addition, the program manager will represent and communicate program updates and performance results at Strive Health in multiple forums, internal and external.

The Day to Day 

  • Become an expert on the CKCC shared savings model and an internal resource for executive leaders, market operators, finance leaders, and other cross-functional teams across Strive Health. For example, this role will develop a deep understanding of program quality measures, patient attribution processes, and financial payment structures.  
  • Monitor critical CMS requirements and timelines to ensure deliverables are completed accurately and on time. This role will also identify and implement best practices for completing these program deliverables across markets. 
  • Educate and work collaboratively with executive leadership, market operations teams, and clinical leaders to build understanding of the model and drive improvements internally and with Strive’s physician partners. This role will also share program learnings and insights to inform Strive’s policy and advocacy work. 
  • Represent Strive Health at CMS webinars, office hours, and other events to gather critical information about program updates and communicate directly with CMS staff. 
  • Collaborate with Strive’s Compliance and Operations teams to ensure adherence to all program policies and procedures. 
  • Review and interpret complex program model documents and communications to guide important operations, finance, legal, and analytics-related activities.  
  • Maintain critical documentation for program participation and own the ongoing submission process to CMS.  
  • Spearhead the strategy and implementation of new program features and available benefits by developing tactical plans, cross-functional relationships, internal alignment, and education.  
  • Support the governance of each market’s Board, including the development of meeting content and coordination of Board approvals. 

Minimum Qualifications 

  • Bachelor's degree in healthcare, Science or related degree.
  • 4+ years related work experience required within a healthcare setting, preferably related to value-based care, a Medicare/Medicaid related program, or other alternative payment model.

Preferred Qualifications  

  • Demonstrated experience with managing or supporting programs related to one of CMS’ accountable care models (e.g. Medicare Shared Savings Program, ACO Reach, Kidney Care Choices). 
  • Understanding of the elements of value-based care models and how organizations participate in these models to drive better outcomes for patients. 
  • Advanced skills with Microsoft applications which may include Outlook, Word, Excel, PowerPoint or Access and other web-based applications.

Annual Base Salary Range: $86,000 - $104,000
