Pennsylvania Licensed RN Care Coordinator

Job Summary

A company is looking for a Registered Nurse Ambulatory Care Coordinator.

Key Responsibilities

• Coordinate implementation of new physician services and refine clinical processes
• Provide clinical support, including anticoagulation management and triaging patients
• Develop and conduct clinical training programs for Access Center staff

Required Qualifications, Training, and Education

• Five to seven years of clinical experience, preferably in Cardiology
• Supervisory experience preferred, with a background in Call Center or outpatient nursing
• Current RN license and Bachelor's degree required
• Provider BLS certification required
• Strong knowledge of managed care and clinical systems preferred
