Job Summary

A company is looking for a Care Navigator.

Key Responsibilities

• Process referrals to secondary care, including specialists and community resources
• Maintain directories of specialists and community services, ensuring timely communication and documentation
• Serve as the primary contact between patients, specialists, and clinic physicians for external care needs

Required Qualifications

• At least 3 years of applicable experience, preferably in a Primary Care setting
• High school diploma or GED required
• Proficient in Google spreadsheets and/or Excel, as well as database management
