Role Description:

Oak Street Health delivers high-quality primary care to Medicare patients in neighborhood centers - and at home when needed. The House Calls Medical Assistant/CIS will fulfill a crucial role, supporting logistics, clinical care, and documentation in our innovative home-based care program. The Home Visit Medical Assistant/CIS will work directly with a House Calls nurse practitioner on a daily basis, in collaboration with center-based care teams, to help identify and bridge gaps in care when patients return from the hospital or experience other challenges.

 

Core functions:

  • Preparing visit schedule, route, and logistics for all visits

  • Accompanying nurse practitioner on all house calls

  • Vital signs, medication reconciliation, patient / family education

  • Documenting House Call note, managing referrals, orders and follow-up

  • Maintaining supplies/equipment

  • Other duties as assigned

Requirements:

  • At least one year experience as a Medical Assistant + MA certification

  • Must pass all clinical competencies before supporting patient care, phlebotomy a plus

  • Daily access to a vehicle with valid license and insurance

  • Flexibility to travel around Chicagoland

  • Bilingual (English and Spanish) a plus

  • US work authorization
