NJ Licensed RN Liaison

Job Summary

A company is looking for an RN Liaison.

Key Responsibilities

• Facilitates patient engagement and transitions across the continuum of care with the interdisciplinary team
• Conducts holistic assessments and develops care management plans in collaboration with care team members
• Documents care plans and patient progress, ensuring effective communication with the care team and families

Required Qualifications, Training, and Education

• Bachelor's Degree preferred
• Valid NJ RN License required; NY RN License preferred
• Minimum of 4 years of Care/Case Management or comparable clinical experience preferred
• Certified Case Manager (CCM) preferred
• Proficient in Microsoft Office 365 and familiar with electronic health records
