Job Summary

A company is looking for a Director of Care Operations.

Key Responsibilities

• Lead and build a high-performing Care Leadership Team while overseeing daily operations to ensure quality patient-centric care
• Collaborate on business and financial planning, define performance expectations, and partner with workforce management to optimize team productivity
• Drive change management initiatives and communicate relevant changes to the Care Team, ensuring proper training and support

Required Qualifications

• 10+ years of experience in the telehealth space, particularly in Chronic Care Management (CCM) and Remote Patient Monitoring (RPM)
• 7+ years of direct leadership experience in managing remote care teams
• 5+ years of experience with forecasting, scheduling, and capacity planning
• Bachelor's Degree required; active healthcare license or certificate preferred (NP or RN)
• Proven change management skills and ability to thrive in a fast-paced environment
