Job Summary

A company is looking for a Community Health Worker to support patients with complex chronic conditions and connect them with community resources.

Key Responsibilities

• Screen patients for Social Determinants of Health needs and connect them with community resources
• Coordinate care with interdisciplinary teams to facilitate clinical orders and prioritize patient needs
• Provide wellness coaching and practical skill development to empower patients in managing their health

Required Qualifications

• Community Health Worker Certification, CMA, RMA, or an Associate's degree in health sciences with 3+ years of healthcare experience
• Experience identifying community resources and assisting patients in accessing them
• Experience working in an interdisciplinary healthcare team
• Ability to advocate for patients with internal and external stakeholders
• Prior experience with complex patients or underserved populations
