Job Summary

A company is looking for a Care Coordinator to support patient care and engagement in a tele-mental health setting.

Key Responsibilities

• Utilize software and local resources to provide necessary information
• Follow up on no-show appointments and assist with rescheduling
• Support patient engagement through phone calls and chat messages

Qualifications

• Background in behavioral health, such as a BS in psychology or social work, or equivalent experience
• Comfortable working in a fast-paced, changing environment
• Self-starter with the ability to work as part of a team
• Experience or comfort with crisis management
• Bilingual in Spanish/Chinese is a plus but not required
