Job Summary

Key Responsibilities

• Provide telephonic patient care, developing and evaluating comprehensive care plans for various disease states
• Educate patients on medication adherence and support their health management through outbound calls
• Collaborate with healthcare professionals to ensure continuity of care for defined patient populations

Required Qualifications

• Registered Nurse with an unrestricted current compact license in Arizona or Texas
• Must reside in Arizona or Texas
• 3+ years of clinical RN experience
• Experience using Microsoft Office applications
• RN Diploma, Associates, or Bachelor's degree in Nursing
