Remote Supportive Care Case Manager (RN)

Job Summary

A company is looking for a Supportive Care Case Manager (RN) to join their team remotely.

Key Responsibilities

• Coordinate care for complex patients, identifying their medical, social, emotional, and financial needs
• Develop and implement care plans in alignment with patient goals and preferences
• Facilitate communication among patients, families, and the healthcare team to ensure quality care transitions

Required Qualifications, Training, and Education

• Minimum of 2+ years experience as an RN in Hospice or Palliative medicine required
• Minimum of five (5) years experience as an RN required
• Current RN License is required; BSN preferred
• Current CPR or BLS certification required
• Intermediate computer skills and proficiency in MS Word, Excel & Outlook required
