Job Summary

A company is looking for a System Manager, Care Transitions.

Key Responsibilities

• Lead initiatives to support the transition to value-based care, focusing on reducing avoidable readmissions and enhancing patient experience
• Coach and support a diverse team to operationalize evidence-based interventions and drive performance in key healthcare areas
• Collaborate with leaders to align care transitions initiatives with broader strategic priorities and enhance continuity of care

Required Qualifications, Training, and Education

• Bachelor's degree in Nursing from an accredited college or university
• Master's degree in Nursing, Healthcare Administration, Business, or a related field from an accredited institution
• Minimum of three years of leadership experience in a healthcare or ambulatory care setting
• Licensure as a registered nurse by the state of South Carolina or compact state required
• Current American Heart Association (AHA) Basic Life Support (BLS) certification or equivalent required
