Job Summary

A company is looking for a Nurse Practitioner to provide remote virtual serious illness care management.

Key Responsibilities

• Provide remote virtual serious illness care management to patients and families
• Consult with medical staff and perform virtual assessments of patient needs
• Document interactions in electronic medical records and participate in quality assurance activities

Required Qualifications, Training, and Education

• Minimum of 3 years of recent clinical experience in hospice, palliative care, or acute inpatient care
• Master's or Doctor's degree in Nursing from an accredited Family or Adult Nurse Practitioner Program
• National certification, licensure, and prescriptive authority in the home state and additional states
• Basic Life Support certificate required by date of hire
• Advanced Certification in Hospice and Palliative Nursing (ACHPN) must be obtained within 3 years of hire
