Job Summary

A company is looking for a Nurse Practitioner- Resource Evening Nights & Weekends.

Key Responsibilities

• Provides fully remote virtual serious illness care management to patients and families across various care settings
• Consults with medical staff and performs virtual assessments to ensure optimal patient care
• Maintains electronic medical records and documents all interactions in a timely manner

Required Qualifications, Training, and Education

• Minimum of 3 years of recent clinical experience in hospice, palliative care, and/or acute inpatient care
• Must hold a Master's or Doctor's degree in Nursing from an accredited program
• National certification, licensure, and prescriptive authority from the appropriate governing body
• Basic Life Support certification required by the date of hire
• Advanced Certification in Hospice and Palliative Nursing (ACHPN) must be obtained within 3 years of hire
