Job Summary

A company is looking for a Manager of Care Navigation (Remote - LCSW/LPC/LMFT Required).

Key Responsibilities

• Supervise and support a remote team of Care Navigators, including hiring, training, and performance management
• Oversee daily operations and optimize processes to ensure high-quality support for patients and caregivers
• Monitor key performance indicators and collaborate with departments to achieve clinical quality metrics

Required Qualifications

• Minimum 3-5 years of experience in healthcare management or care coordination leadership
• Possess a current, unrestricted clinical license: LCSW, LPC, or LMFT
• Preferred experience working with electronic health records
• Experience managing remote teams is strongly preferred
• Motivated to provide excellent experiences for patients, caregivers, and providers
