Job Summary

A company is looking for a Pharmacy Care Coordinator.

Key Responsibilities

• Act as a liaison between members, providers, and pharmacies or care facilities
• Coordinate the scheduling of patient medication delivery and process patient order requests
• Verify patient information, enter data into the pharmacy software system, and prepare prescriptions for pharmacist review

Required Qualifications

• Must be able to stand/sit for long periods of time within limited space
• Must be able to lift up to 25 pounds
• Experience in customer service and teamwork
• Detail-oriented with the ability to work independently and meet deadlines
• Ability to work cooperatively with pharmacy services and staff
