Pennsylvania Licensed Physician Partner

Job Summary

A company is looking for a Physician Partner (PA).

Key Responsibilities

• Conduct telehealth visits with patients, providing clinical guidance and prescribing medications as appropriate
• Collaborate with Community Health Workers (CHWs) to assess patient needs and facilitate access to healthcare services
• Maintain patient records and provide oversight to CHWs in addressing social needs and healthcare navigation

Required Qualifications

• Active, unrestricted MD or DO license; NP licenses also accepted
• Minimum of 2 years of experience in family medicine, internal medicine, geriatrics, or preventative medicine
• Commitment to health equity and social medicine
• Ability to operate independently with strong diagnostic skills
• Experience in telemedicine and proficiency with relevant technology
