Job Summary

A company is looking for a Triage & Transitions Ambulatory Registered Nurse.

Key Responsibilities
  • Manage patient calls requiring triage and coordinate care during transitions post-inpatient stay
  • Provide guidance, education, and resources for managing at-risk patient populations
  • Perform clinical triage, including assessments and patient education using established protocols
Required Qualifications, Training, and Education
  • ASN/ADN from an accredited program with the ability to obtain a BSN within four years or 15 years of RN experience in acute care
  • Current Montana RN license or compact license with the ability to obtain an RN license in the state of practice
  • Current BLS certification endorsed by the American Heart Association
  • Preferred: Bachelor's Degree in Nursing (BSN) from an accredited institution
  • Preferred: One year of experience in a clinic or inpatient acute hospital setting
