Michigan Licensed Care Manager (RN)
Job Summary
A company is looking for a Care Manager (RN).
Key Responsibilities
- Evaluate member needs and recommend care plans to address barriers to accessing appropriate care
- Develop and coordinate ongoing care plans in collaboration with healthcare providers and community resources
- Monitor member progress and provide education to members and their families regarding healthcare options and benefits
Required Qualifications
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
- 2 - 4 years of related experience in nursing
- Licensed as a Registered Nurse in Michigan or a Compact State
- Experience in physical health and obstetrical nursing is preferred
