Job Summary

A company is looking for a Director of Central Care Operations.

Key Responsibilities

• Establish and execute centralized patient enrollment and engagement operations
• Design and deploy scalable operational processes for patient care and practice engagement
• Manage the Patient Enrollment Specialist and Health Engagement Specialist teams

Required Qualifications

• 5-7 years of experience in care management or patient recruitment within a provider or health plan setting
• Experience with process design and change management
• Understanding of population health management from provider or payor perspectives
• Familiarity with common EMRs such as Athena, Epic, and eClinicalWorks
• Ability to operate in an everchanging environment
