Job Summary

A company is looking for a Care Coordinator I to facilitate a seamless experience for clients and clinicians in mental healthcare.

Key Responsibilities

• Facilitate client care coordination and assist with scheduling needs
• Provide administrative support to clinicians and ensure smooth communication across teams
• Monitor client progress and follow up to ensure care plans are executed effectively

Required Qualifications

• Previous experience in a medical/mental health office with scheduling and referral coordination
• Working knowledge of Electronic Medical Records Systems
• Experience with Excel, Word, and other computer programs
• Ability to work effectively in a team with minimal supervision
• Customer service experience (2 years preferred)
