Job Summary

A company is looking for a Care Coach to support adult patients in their healthcare journey.

Key Responsibilities

• Conduct health assessments and coordinate care for adult patients, particularly the elderly or chronically ill
• Guide patients through their healthcare journey and ensure they understand their health benefits
• Collaborate with healthcare team members to provide appropriate care at the right time

Required Qualifications, Training, and Education

• Bachelor's degree in allied sciences, social sciences, social work, human services, or a related field (Profile 1) or any discipline (Profile 2)
• Minimum of 1 year (Profile 1) or 2 years (Profile 2) of direct adult patient care experience
• Proficiency in Microsoft Word, Excel, Outlook, and electronic records systems
• Ability to build trust with patients and healthcare stakeholders
• Critical thinking and problem-solving mindset
