Job Summary

A company is looking for a Bilingual Spanish and English Pre-Visit Care Connector.

Key Responsibilities

• Completes telephonic outreach to members and receives inbound calls
• Influences members to complete the Pre-Visit Assessment and documents necessary information
• Ensures members have access to Primary Care Providers and identifies gaps in care

Required Qualifications

• High School Diploma / GED or equivalent work experience
• 2+ years of call center and/or telephonic customer service experience, preferably in healthcare
• 2+ years of healthcare experience or social work/community outreach experience
• Bilingual fluency in Spanish and English
• Experience using Microsoft Word, Excel, and Outlook
