Key Responsibilities

• Provide triage assessments and support to patients and families
• Coordinate care and communicate effectively with healthcare teams
• Document patient information and maintain accurate records

Required Qualifications, Training, and Education

• Graduated from an approved school of professional nursing
• Licensed to practice as a registered nurse in the state of agency operation
• Experience in hospice or palliative care is preferred
• Basic computer skills required
• Availability to work weekends and one weeknight shift
