Job Summary

A company is looking for a Licensed Practical Nurse (LPN) to support Chronic Care Management and Remote Patient Monitoring programs.

Key Responsibilities

• Conduct comprehensive patient assessments and identify barriers to effective disease management
• Assist in developing and implementing individualized care plans in collaboration with a multidisciplinary team
• Educate patients on chronic disease management and facilitate communication among healthcare team members

Qualifications and Requirements

• Active TENNESSEE Licensed Practical Nurse (LPN) license in good standing
• At least 2 years of experience in patient care, care coordination, or chronic disease management
• Proficiency in electronic health records (EHR) systems
• Certification in disease management, case management, or patient education is a plus
• Reliable internet connection and a HIPAA-compliant workspace for telephonic and virtual interactions
