Job Summary

A company is looking for a Care Manager Transition of Care.

Key Responsibilities
  • Evaluate member needs through post-discharge assessments and medication reconciliation
  • Develop care/service plans in collaboration with various healthcare providers to support member transitions
  • Facilitate communication and services among medical and non-medical resources to address member needs
Required Qualifications
  • Master's degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing
  • 2 - 4 years of related experience in care management
  • Licensure as LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN is required
  • Experience with compliance to state, federal, and third-party payer regulations
  • Ability to provide education and resources to members and caregivers
