Key Responsibilities

• Use motivational interviewing to empower members in achieving their health and personal goals
• Conduct outreach to understand member needs and provide personalized support
• Identify and refer members to local resources and services for comprehensive support

Required Qualifications

• 2+ years of experience in Care Navigation, Social Work, Health Coaching, or related fields
• Fluency in Spanish is required
• Experience with Motivational Interviewing or similar supportive approaches
• Familiarity with Medicaid, SNAP, and public housing systems
• Experience working with low-income individuals or those with chronic conditions
