Job Summary

A company is looking for a Contract Nurse Practitioner (Remote).

Key Responsibilities

• Conduct video and phone visits for chronic care management and remote patient monitoring
• Perform care coordination and recommend cost-effective treatment interventions
• Monitor the quality of member care and update care plans as necessary

Required Qualifications

• Certified and licensed as a Nurse Practitioner in good standing in relevant states
• Bilingual in English and Spanish (preferred)
• 2+ years of clinical experience with complex adult populations
• 1+ years of telephonic triage or equivalent experience
• Master's or doctoral degree from an accredited institution for nurse practitioners
