Washington State Care Manager

Job Summary

A company is looking for a Care Manager Transition of Care - Discharge.

Key Responsibilities

• Evaluate member needs and complete post-discharge assessments for those transitioning from healthcare facilities
• Develop care/service plans and collaborate with various healthcare teams to support member transitions
• Facilitate education and resources for members and caregivers to promote overall wellness and improve care quality

Required Qualifications

• Master's degree in Behavioral Health or Social Work, or a degree from an accredited School of Nursing
• 2 - 4 years of related experience in care management
• Licensure as LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN is required
• Residency in Washington State is required for this remote position
