Job Summary

A company is looking for a Case Manager DC.

Key Responsibilities

• Support the team and families by reviewing assessments, care plans, and supervision requests for Medicaid Waiver recipients
• Ensure compliance with documentation and justification requirements for services provided
• Maintain organized records and manage electronic tools for remote work

Required Qualifications, Training, and Education

• Documented combination of relevant skills and work experience
• Person-Centered Thinking/Planning training is a plus
• Knowledge of Virginia Medicaid Waivers is advantageous
• Must have reliable internet connection and experience with various applications including Office, Google, and Adobe
• Must pass a criminal background check and provide two references
