Job Summary
A company is looking for a Lead Care Manager RN.
Key Responsibilities
- Oversees and reviews ongoing care plans and collaborates with providers to address member needs
- Provides guidance and expert knowledge to the care management team to ensure quality control and adherence to policies
- Monitors member status and outcomes, revising care plans as necessary based on identified needs
Required Qualifications
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
- 5 - 7 years of related experience
- RN - Registered Nurse - State Licensure and/or Compact State Licensure required
