Job Summary

A company is looking for a Population Health Coordinator.

Key Responsibilities

• Manage data and registries for special populations, facilitating care coordination and quality improvement
• Support local care teams with complex member care and assist in training staff on the Collaborative Care Model
• Provide direct member care, including screenings, assessments, and education on health-related issues

Required Qualifications

• Medical Focus: RN or LPN with SAMHSA X DEA license training, Bachelor's degree in Nursing preferred, and 5 years of relevant experience
• Mental Health Focus: Current licensed clinical social worker with 5 years of experience in mental health care
• Social Care Focus: Current social worker or certified peer/community health worker with 5 years of experience in case management
• Proficiency with EMRs and Microsoft Office/Google Suite
• Access to reliable internet and telephone services for remote roles
