Job Summary

A company is looking for a Manager, Clinical Care Case Management.

Key Responsibilities

• Manage day-to-day operations of Health Plan Clinical Programs to enhance clinical efficiencies
• Oversee case management and transitional care for patients, ensuring appropriate follow-up services are provided
• Implement and direct health education activities for beneficiaries, providers, and staff

Required Qualifications

• RN/BSN required
• 5 years of management experience
• 4 years of clinical nursing experience
• At least 4 years of extensive experience in chronic care case management in an outpatient setting
• Certification in case management preferred
