Job Summary

A company is looking for a Care Navigator- Licensed Nurse.

Key Responsibilities

• Manage physiological markers like blood pressure, weight, and blood glucose with clinical appropriateness
• Provide preventive health and disease management education and coaching through wellness calls
• Collaborate with patients and their care team to define health goals and manage patient messaging and alerts

Required Qualifications

• Unrestricted RN/LVN/LPN license in a compact state and New York or Massachusetts
• 1-3 years of clinical experience, patient management, or disease management desired
• Experience with Medicare patients and performing virtual visits or telephonic care management
• Ability to work independently with minimal direction in the EST time zone
• Spanish speaking is a plus
