Job Summary

A company is looking for a Telephonic RN Nurse Care Manager.

Key Responsibilities

• Coordinate care services with the pod leader to ensure comprehensive patient access to tailored services
• Develop and manage personalized care plans, addressing care gaps and ensuring access to necessary resources
• Perform nursing assessments via telephone and provide education on disease management and health maintenance

Required Qualifications

• Active Registered Nurse License
• 2+ years of care management experience in community, health plan, or hospital systems
• Strong clinical skills and proactive thinking
• Knowledge of Medicare regulations and home care standards
• Bachelor of Science in nursing or related field preferred
