Job Summary

A company is looking for a Care Manager (RN) to provide case management for members across multiple states.

Key Responsibilities

• Evaluate member needs and develop personalized care plans
• Coordinate care between members, families, and healthcare providers
• Monitor member status and update care plans as necessary

Required Qualifications

• Degree from an Accredited School of Nursing or Bachelor's degree in Nursing
• 2 - 4 years of related experience in nursing
• Registered Nurse (RN) State Licensure and/or Compact State Licensure
• 3+ years of clinical nursing experience in acute care settings is strongly preferred
• 2+ years of case management experience in a managed care environment is a plus
