Job Summary

A company is looking for a Community Health Worker.

Key Responsibilities

• Conduct Access to Care assessments with KY Medicaid members via phone
• Accurately document responses in the Community Care system
• Help close care gaps related to Social Needs Screening - Enhanced (SNS-E)

Required Qualifications

• Background in community health, healthcare, or Medicaid
• Prior CHW or healthcare experience preferred
• Familiarity with KY Medicaid and social determinants of health (SDOH)
• Organized and tech-savvy
• Proficient in motivational interviewing and culturally competent care
