Compact Licensed Disease Management Nurse

Job Summary

A company is looking for a Disease Management Nurse - Remote.

Key Responsibilities

• Provide telephonic and omni-channel support to participants in the Disease Management program
• Educate participants on their chronic conditions and assist in developing personalized care plans
• Identify at-risk participants and facilitate necessary referrals to healthcare providers

Required Qualifications

• Current Registered Nurse multi-state compact license; BSN preferred
• Minimum of 3 years recent clinical experience
• Basic knowledge of physiology and chronic diseases relevant to nursing
• Ability to effectively communicate with members and healthcare professionals
• Basic computer skills with a typing speed of at least 40 WPM
