A company is looking for an LPN Care Coordinator.

Key Responsibilities

• Assess and document patients' socio-economic reviews to support care coordination
• Plan interventions to help patients navigate the health system and reduce barriers to care
• Participate in huddles with primary care practices to assist with care coordination and program metrics

Required Qualifications

• Must live in Colorado
• Experience in primary care office settings
• Knowledge of care management and social determinants of health
• Familiarity with value-based care and quality metrics
• Experience with Epic/Electronic Medical Records and Microsoft Office
