Job Summary

A company is looking for a Care Coach I to empower caregivers through support and education.

Key Responsibilities

• Engage and educate caregivers using phone and digital technology to manage their health and wellbeing
• Support multiple caregiver programs, focusing on resource navigation and care coordination
• Collaborate with the coaching team to address caregiver challenges and document interactions accurately

Required Qualifications

• 2+ years of relevant experience in care or disease management, particularly with older adults or individuals with disabilities
• Comprehensive knowledge of Medicaid, Medicare, and Managed Care
• Strong working knowledge of case management systems and community service delivery
• Fluency in Spanish, both written and spoken
• Ability to multitask and make decisions in a fast-paced environment
