Arizona Licensed Ambulatory Pharmacist

Job Summary

A company is looking for an Ambulatory Clinical Pharmacist.

Key Responsibilities

• Provide high-quality direct patient care and serve as a drug information specialist
• Conduct medication therapy management and adjust medication regimens to optimize patient outcomes
• Document patient assessments and clinical interventions accurately and completely

Required Qualifications

• A degree in Pharmacy from an accredited College of Pharmacy
• Completion of an accredited PGY1 pharmacy residency program or three years of pharmacy experience
• Current and unrestricted pharmacy licensure in the applicable state of practice within 90 days of hire
• Good organizational and computer skills
• Good written and verbal communication skills for interfacing with staff and patients
