Job Summary

A company is looking for a Population Health Registered Nurse.

Key Responsibilities

• Conduct telephonic outreach to patients for post-discharge assessments and education
• Facilitate care coordination between healthcare services to enhance patient outcomes
• Collaborate with internal teams to improve population health outcomes

Required Qualifications

• 2+ years of experience in case management or as a primary care RN with population health experience
• Experience engaging with Medicare and Medicaid populations with complex chronic conditions
• Nurse Licensure Compact (NLC) required; active RN license(s) in specific states preferred
• Ability to leverage motivational interviewing skills to encourage patient behavior changes
• Familiarity with Population Health and HEDIS quality gaps in care preferred
