Registered Nurse Cardiac Coach

Job Summary

A company is looking for an RN Cardiac Health Coach - Wellness.

Key Responsibilities

• Complete health assessments and implement care plans for members with chronic conditions
• Provide telephonic support and coaching on wellness care plans to improve members' health
• Monitor member status and outcomes, making recommendations to reduce health risks

Required Qualifications, Training, and Education

• Associate's degree with 2 - 4 years of related experience
• Must be a Registered Nurse with cardiac experience
• Residency in one of the specified states (Indiana, Iowa, Kansas, Nebraska, New Hampshire, North Carolina, South Carolina)
• Relevant certifications such as RN, Master's in Behavioral Health, or Health Coach Certification required
• CDCES certification required upon hire for Enterprise Population Health
