Job Summary

A company is looking for a Clinical Documentation Specialist - Physical Therapist.

Key Responsibilities

• Review OASIS documents and key elements of the Plan of Care for CMS compliance
• Collaborate with Quality Improvement Manager and Home Health leadership to ensure regulatory compliance and positive patient outcomes
• Serve as a role model and OASIS expert for Home Health staff across the region

Required Qualifications

• Education sufficient for licensure
• Oregon Physical Therapist License upon hire
• 2 years of clinical experience as a Physical Therapist, or 1 year in Home Health, or 1 year of proficiency in Home Health OASIS Review
• 3 months of experience with an Electronic Medical Record (EMR) system
