Ohio Licensed LTSS Coordinator

Job Summary

A company is looking for an LTSS Coordinator II to monitor and promote effective utilization of long-term services and supports through clinical review and benefits management.

Key Responsibilities

• Act as a point of contact within the interdisciplinary care team to support coordination of long-term services and supports
• Complete reviews for NF level of care determinations, respite care, waiver services, and home modifications
• Coordinate care with internal and external stakeholders to facilitate timely and cost-effective discharge planning

Required Qualifications, Training, and Education

• Completion of an accredited Registered Nurse (RN) degree program or degree required for Social Worker licensure in Ohio
• Bachelor's Degree in Nursing (BSN) or equivalent healthcare degree is preferred
• Minimum of three (3) years of experience in case management and/or managed care is required
• Experience in a Medicare and/or Medicaid managed care environment is preferred
• Current, unrestricted Registered Nurse (RN) or Licensed Social Worker (LSW) licensure in Ohio is required
