Job Summary

A company is looking for an Employee Health Plan Case Manager to manage patient care and outcomes remotely.

Key Responsibilities

• Coordinate care and collaborate with providers to manage patient outcomes effectively
• Conduct ongoing assessments and evaluations to monitor patient progress and needs

Required Qualifications

• Graduate from an accredited school of nursing
• Ohio licensure as a Registered Nurse (RN)
• Basic Life Support (BLS) certification through the American Heart Association (AHA)
• Three years of relevant clinical experience
• Knowledge of medical terminology and coding standards
